Once the MSA is complete, you must attach a Certificate of Insurance naming the Client as Additional Insured and the Certificate Holder, in accordance with the coverage limits outlined in the MSA document. Failure to do so will prevent your company from being added to the Milwaukee Transit System Program vendor list. Please reference the sample certificate for the standard insurance requirements for Milwaukee Transit System.
On the COI, complete the Certificate Holder field as follows:

9800 Crosspoint Boulevard

Step 3: W-9 Form

In addition to the COI, a W-9 form must be completed, signed, and attached to the MSA at the time of submission. -
Step 4: HireRight Registration
Once your company is registered in the program, the next step is to register with the required background screening company - HireRight. Please email mcmsp@knowledgeservices.com with the name and contact information of the person who has the authority to register and establish an account.
The contact will receive a registration email to register with HireRight. Registration will require a nominal set up fee ($84.40 if not already currently using HireRight), the completion of a subscriber agreement and a mandatory credentialing process for compliance purposes which will take 5-7 days. Delays in registering for your account may lead to exclusion from the bidding process for opportunities at Milwaukee Transit System.
